NOTE: All information regarding the program is sent via email. Please provide one current email address that you would like to use for this purpose; preferably the email of the parent most involved with the program and one that is checked frequently. You will have a chance to enter additional email addresses upon registering for classes.

Dancers and their parent/guardian agree to the following:

I understand that I am required to enroll in & attend: Summer Term, Medley Team Bootcamp (if applicable), Fall/Winter Term, and ALL REHEARSALS, COMPETITIONS, & PERFORMANCES. I commit to full participation during the entire term from June until the end of May.

I understand that I may be expected to take private lessons, attend master classes, and practice as part of my dance training regimen.

I understand that I may be required to compete with a partner throughout the year. I understand that it is not ISD’s responsibility to find me a partner.

I commit to scheduling myself in a way that will not compromise my commitment to the team.

I understand that proper dance clothes must be worn to every class, and I commit to adhering to ISD’s dress code and my team dress code.

I have read and understand ISD’s Code of Conduct and I commit myself to abiding by it while I am a member of the ISD Ballroom Company.

Parent Commitment(Required)

I have read and understand Island Style's Parent Booster Club policy and I am willing and able to either: fulfill my required volunteer hours throughout the year OR pay the "Volunteer Opt Out Fee" of $10 per required hour.

I understand that all communication from the coach / studio will be via email, text messages, and/or the GroupMe messaging app. It is my responsibility to be updated on all studio and team communication, including keeping track of all dates and team expectations as I am informed.

I understand that formation team dancing is competitive and is a physically and emotionally demanding activity. I am confident in my dancer’s physical and emotional ability to cope with all aspects of team commitment and participation for the full term.

I commit to attend all required parent meetings.

I have reviewed the tuition and team fee information and I AM WILLING AND ABLE TO PAY ALL FEES associated with being on a ballroom team for the whole term – including tuition, team fees, travel fees, and all other fees associated with the team and the classes in which my student is enrolled. I understand that all fees must be paid in order for my dancer to participate in performances and competitions. I understand that no refunds will be given for tuition or team fees, etc.

I have read and understand the studio TUITION / FEE POLICY and acknowledge that ISD reserves the right to drop my dancer from the team if tuition and/or fees are more than 30 days overdue.

I understand that the annual team fee is due in full no later than September 15. I understand that a $50 late fee will be assessed if the team fee is not paid in full by then, unless I have previously worked out a payment plan with ISD’s account manager.

I am willing and able to purchase all items on the list of supplies needed contained on my TEAM PAGE (located on the Ballroom Team Portal) and all supplies that may be necessary for their participation on an ISD team.

Transportation Policy:
By initialing this box and signing this team contract, I acknowledge that I have read and understand ISD’s Policies & Fees located on www.islandstyledance.com. I have given full permission as the parent/legal guardian (if participant is a minor) to, and/or have voluntarily agreed to be a part of a team that will travel to and from events (performances, competitions, studio-related events); within or without the state of Utah. It is my express intent that this waiver of liability acknowledges that I understand and agree that Island Style Dance Studio & Performing Company, LLC (ISD) assumes no liability, and I hold ISD harmless for my transit/the transit of my child, or for incidents related to my transit/the transit of my child while traveling in a private vehicle. I further agree that I/my child is not in the care, custody, and/or control of ISD while traveling to and from events in a private vehicle. I understand that ISD does not provide health and/or accident insurance for trip participants and I understand that any medical expenses, property loss, or other personal expenditures that result during or from travel/trips are the responsibility of the student/participant or by their parent/guardian (if the participant is a minor). I also hereby consent, give authorization to, and release from liability, Island Style Dance Studio & Performing Company, LLC, studio director, staff, and all trip-related leaders in securing any emergency medical treatment in the event I am unable to, and I agree to be responsible for the costs thereof. By initialing the box and signing this team contract I acknowledge that with full understanding of the above issues, conditions and risks, I hereby release, indemnify, and hold harmless Island Style Dance Studio & Performing Company, LLC, its director/president, faculty, staff, trustees, officers, volunteers, coaches and agents from all form and manner of risks inherent in, and from all claims, suits and demands of any nature arising from participation in any events that require travel.

Parents:
By signing this contract, you agree to abide by all expectations and commitments, you give permission for your son(s) and/or daughter(s) to be a team member of the designated team, and you allow your child to participate in all events and performances. You acknowledge that you trust your child’s ability to act appropriately for their own safety at and during an event, performance, and/or competition. You acknowledge that ballroom dance / cabaret can be very physically demanding and that Island Style Dance Studio & Performing Company, LLC will provide all reasonable precautions to ensure the proper training and safety of each student. However, because of the physical demands ballroom dance / cabaret place on the body; sometimes injuries do occur; therefore, you hereby release Island Style Dance Studio & Performing Company, LLC, its Presidents, Managers, independent contractors, assistants, student teachers, and instructors from any and all liability that might result from ballroom dance / cabaret-related injuries. You understand that Island Style Dance Studio & Performing Company, LLC, its presidents, manager, director, instructors, agents, and all independent agents contracted by ISD are not liable for loss or injury due to participation.

Team members:
By signing this contract you agree that you are responsible for yourself at and during every event, performance, and/or competition and will follow the standards and commitments set forth in this contract and at the direction of the director and/or coaches for the safety of all team members and yourself.
